Missed doses & double dosing

What to do if you miss a dose or can’t remember taking one, and why you should never double up.

The one rule: never double up

Never take two doses to catch up on a missed one. A double dose doesn’t speed up results; it raises the side effects. With the weight-loss peptides that can mean days of vomiting and dehydration, which is exactly how people end up in the emergency room.

Not sure whether you took it?

Don’t take another. Skip it and take your next scheduled dose. Missing one dose is far safer than taking two.

Weekly weight-loss peptides

  • Semaglutide: take the missed dose if your next one is more than 2 days away. Otherwise skip it and carry on as normal. (This is what its approved label says.)
  • Tirzepatide: take it if you are within 4 days of the missed day. Otherwise skip it. Always keep at least 3 days between doses. (Also from its label.)
  • Retatrutide: there’s no approved label yet, so follow the tirzepatide rule.

After a longer gap, two weeks or more, your stomach’s tolerance can drop. Restarting at a lower dose and building back up is the cautious approach; check with a clinician.

Daily and 5-on / 2-off peptides

Skip the missed dose and take the next one at your usual time. Never two in one day.

Two or three times a week

For NAD+, MOTS-C, Semax, Selank and Thymosin Alpha-1: if you remember the same day, take it. Otherwise skip it and keep to your next scheduled day, with at least a day between doses.

As needed

PT-141: no more than one dose in 24 hours. Its approved form also limits use to 8 doses a month.

Never wonder again

  • Use the dose log on the calculator. It warns you if you try to log a second dose too soon.
  • Set up dose-day emails. Each one has an “I took it” button, and pressing it twice tells you it’s already done.
  • Take it on the same day and at the same time each week, and tie it to something you already do.

If you think you took two doses

Don’t take any more. Call Poison Control (1-800-222-1222 in the US) or your clinician for advice, and watch for the warning signs in side effects: normal vs. urgent. Vomiting that won’t stop, not keeping fluids down or severe stomach pain means get seen today.
